Click here to download the Corporate Information Form Monthly Income & Expense Statement Individuals who have declared bankruptcy are required to complete and submit monthly income & expense forms to their Trustee. Income and Expense forms are required to be accompanied by copies of your monthly pay stubs. Click here to download the Monthly Income and Expense statement. Proof of Claim Form Creditors use this form to prove their claim in a bankruptcy/proposal estates. Creditors who have proven their claim are permitted to participate in the estate and are eligible to receive a pro-rata share of any dividend distributions. Click here to download the Proof of Claim Form Proxy Form Creditors can use this form to appoint an individual for the purpose of attending meetings, voting and discuss important issues on their behalf. A proxy can be used by an individual creditor to appoint another individual to attend a meeting or by a corporate creditor to appoint an individual to attend a meeting on the corporations behalf. Creditors considering appointing a proxy must recognize only an individual can be appointed proxy, a corporation can not be appointed as a valid proxy.
